In order to make things as safe as possible & to minimise risk of ongoing transmission of Covid-19, it is important that you read the following information & discuss any queries with me prior to attending for your appointment.
When you arrive
If arriving by car, please park on the drive. DO NOT press the doorbell but text or ring me on 07917 765194.
I will come out & greet you via the side entrance gate. It's important to ask you some Covid-19 related questions, prior to you entering the clinic room.
Please ensure you read through the following compliance prior to attending your appointment as you will be required to confirm your compliance with my new conditions with your signature on entering the clinic
Do you (or anyone in your household/bubble) currently have any of the following symptoms:
1) A new, continuous cough (this means coughing a lot for more than an hour, or 3 or more coughing episodes in 24 hours; if you usually have a cough, it may be worse than usual)
2) High Fever (this may mean you feel hot to the touch on your chest or back, more than usual)
3) Loss or change of your sense of smell or taste (you may have noticed you cannot smell or taste anything or things smell or taste different to 'normal')
4) Have you ever tested positive for Covid-19?
If you answer yes to any of these questions then please re-book for another time or ring me to discuss
Are you in an identified high-risk group or shielding someone in a high-risk group (please get in contact with me prior to your visit if you answer 'yes' to this question so we can discuss all risks)
In the past 14 days, have you:
a) Been outside the UK (this is acceptable providing you have been to an air-bridge compliant country but I will need to document this so please tell me)
b) You or anyone in your family/bubble been told to self-isolate for any reason (by the NHS Test & Trace/GP) or just precautionary self isolation
c) Have you or any of your household been in contact with a confirmed positive case in the last 14 days
d) Had a test positive or negative for Covid-19 (you have had an official NHS test, please provide details of the outcome of the test & the date of your test
Clients should be aware that their details will be released for the Governments contact Track & Trace if required
I am supporting the NHS Track & Trace App & have a QR code available for you to check-in with on your arrival.
I will temperature check you with a contact-less thermometer prior to entry on to the premises
It is mandatory for you to wear a face covering when in the clinic room (unless you have a medical reason that prevents you from wearing one)
I will be wearing appropriate PPE for the relevant government 'tier' we are in during your treatment
Only bring essential personal belongings into the clinic
I encourage everyone to stay hydrated during a session but at this time please bring in your own water bottles for consumption.
The clinic toilet is available for client use but I would ask you to only use this when necessary.

I confirm that I have read & understand the information provided on Smoothing Knots web page, sent in a link from Smoothing Knots prior to my appointment & have acknowledged this with written communication (text/e-mail)
The information I have provided is honest, accurate and correct to the best of my knowledge.
I have had the opportunity to ask all the questions about its content & all of my questions have been answered to my satisfaction.
I appreciate that although all reasonable steps to reduce risk of infections have been taken, including screening potential Covid-19 cases & undertaking increased hygiene & distancing protocols, there may still be a risk of infection from face to face treatment.
I knowingly & willing consent for Face to Face appointment to take place.
Data Protection Policy
Smoothing Knots (SK) fully complies with the latest Data Protection Policy & has a transparent approach to Data Processing which empowers individuals to know about the collection & use of their personal data. SK collects data for ensuring it has the right information for assessing your suitability to treatment, completing the appropriate treatment, contacting you regarding appointments &, if necessary, for a referral to GP or other healthcare practitioners if deemed necessary.
In addition, the data will be shared with NHS Trace & Test (mandatory) if required, to minimise the spread of Covid-19. SK has the right to refuse treatment f such data cannot be collected. SK only collects data that is relevant to those purposes & will retain this data for 7 years. All information held will be treated as strictly confidential & will only be released to any other external party with the full consent of the client."
I have read The Clinic’s Data Protection Policy & consent to Smoothing Knots processing records as outlined above. I also understand that I can withdraw my consent on the processing of data at any time via written communication.
