In order to make things as safe as possible & to minimise risk of ongoing transmission of Covid-19, it is important that you read the following information & discuss any queries with me prior to attending for your appointment.

When you arrive

If arriving by car, please park on the drive and text or ring me on 07917 765194 and I will come out & greet you via the side entrance gate.  I will take your temperature prior to you entering the premises. 

Please ensure you read through the following compliance prior to attending your appointment as you will be required to confirm your compliance via text/mail before you attend the clinic.

If you have any of the following symptoms on the day of your visit, please re-schedule your appointment 

1) A new, continuous cough 

2) High Fever (this may mean you feel hot to the touch on your chest or back, more than usual)

3) Anosmia - Loss or change of your sense of smell or taste 

In the last 48 hours have you had a Covid-19 vaccination (first or second, any type).

YES? Please re-book as our industry Association recommends not having a treatment within this time period, to allow for any potential side effects from the vaccination to pass. Thanks.

If you answer yes to any of these questions then please re-book for another time or ring me to discuss

Please let me know if in the past 14 days you have:

a)    Tested positive for Covid-19?

b)    Have any symptoms of 'Long Covid' that I should be aware of?

c)    Been outside the UK

If you have recently travelled, this is NOT an issue but I do need to know. The UK governments country 'traffic-light' system is chaotic so providing you can demonstrate a negative test result (as per government guidelines), there should be no issue in attending the clinic

d)   You have been told to self isolate for any reason (by the NHS Test & Trace/GP) or want to be precautionary

Clients should be aware that their details will be released for the Governments contact Track & Trace if required

It is no longer mandatory for you to wear a face covering when in the clinic room but if you have NOT been DOUBLE vaccinated, please do wear a Face Mask. 

As always, I will be wearing appropriate PPE in compliance with the latest government recommendations during your treatment

I encourage everyone to stay hydrated during a session but at this time please bring in your own water bottles for consumption..... which reminds me, the clinic toilet is available for client use!

The information I have provided is honest, accurate and correct to the best of my knowledge.

I have had the opportunity to ask all the questions about its content & all of my questions have been answered to my satisfaction.

I appreciate that although all reasonable steps to reduce risk of infections have been taken, including screening potential Covid-19 cases & undertaking increased hygiene & distancing protocols, there may still be a risk of infection from face to face treatment.

I knowingly & willing consent for Face to Face appointment to take place.


Data Protection Policy

Smoothing Knots (SK) fully complies with the latest Data Protection Policy & has a transparent approach to Data Processing which empowers individuals to know about the collection & use of their personal data. SK collects data for ensuring it has the right information for assessing your suitability to treatment, completing the appropriate treatment, contacting you regarding appointments &, if necessary, for a referral to GP or other healthcare practitioners if deemed necessary. 

In addition, the data will be shared with NHS Trace & Test (mandatory) if required, to minimise the spread of Covid-19.  SK has the right to refuse treatment f such data cannot be collected.  SK only collects data that is relevant to those purposes & will retain this data for 7 years. All information held will be treated as strictly confidential & will only be released to any other external party with the full consent of the client."
